Observed stove, in food preparation area, with an accumulation of dust, dirt, food residue, debris on nonfood contact surfaces. Clean beverage station. Clean floors in produce walk-in refrigerator. Observed broken glass under hand-wash sink in bar area. Clean inside of all cold hold units. Clean vents in food preparation area next to office. Thermometers for ensuring proper temperatures of food are not available or readily accessible for monitoring the temperature of cooked food. Grime observed in the hand-wash sink in the restroom. The hand-wash sink in the kitchen was blocked by a large trashcan and not accessible at all times for employee use. Soap was not available at the hand-wash sink in the restroom. The ceiling in the back storage area has two large holes in it with a large crack that runs across the ceiling between both holes.
