The Pennsylvania Department of Agriculture, 866-366-3723, uses a risk-based inspection reporting process for restaurants and other food handlers.

Big Mike’s Beer Outlet, 465 Penn Ave. Cleona, May 17. Pass. Plumbing system not maintained in good repair. Hot water faucet at the 3-compartment sink was observed in disrepair.

Fresh Donuts Shop, 1202 Cumberland St. Lebanon, May 17. Pass. Single-use disposable gloves are intended for single-use. Facility hangs gloves on a shelf to be used again. Five-gallon white pails of garbage are stored on the ground behind the facility while waiting for someone to get them. One of the pails was open and filled with water thereby becoming a habitat for mosquitos and other pests.

Subway Sandwiches, 104 Northside Commons, Palmyra, Complaint, May 16. Pass. No violations.

World War Memorial Association, 975 E. Main St. Palmyra, May 16. Pass. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Non-food contact surfaces (hood filters) not cleaned at a frequency to preclude accumulation of grease.

Fifth Ward Athletic Club, 750 15th Ave. Lebanon, May 15. Pass. Thermometers for ensuring proper temperatures of food are not available or readily accessible. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Can opener blade, a food contact surface, was observed to have old food residue and was not clean to sight and touch. Non-food contact surfaces (hood filters) not cleaned at a frequency to preclude accumulation of grease.

Sunset Outlet, 1650 N. 7th St. Lebanon, May 15. Pass. No violations.

Cumberland Club, Inc., 713 Locust St. Lebanon, May 14. Pass. No sign or poster posted at the handwash sinks in the toilet rooms to remind food employees to wash their hands.

Price Rite #408 Lebanon Valley Mall, 2231 Lebanon Valley Mall, Lebanon, May 14. Pass. No violations.

Quentin Grove, 2395 Quentin Rd. Lebanon, May 14. Pass. No violations.

M&B Pizzeria Retaurant, LLC, 701 Lehman St. Lebanon, Follow-up, May 13. Pass. Scoop being stored in the ice machine with handle touching the ice. Removed. Two items were marked with the date of 5/9, which was one day prior to this inspection; however, these containers also had a previous date, 4/25, that had not been removed from the container. The interior of the ice machine, a food contact surface, was observed to have dark residue and was not clean to sight and touch. One spray bottle of degreaser was not marked with the name of the item. Corrected during the inspection.
