The Pennsylvania Department of Agriculture, 866-366-3723, uses a risk-based inspection reporting process for restaurants and other food handlers.

Dollar Tree # 06779, 110 Old York Rd. Suite 214, New Cumberland, May 18. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed numerous cases of food items stored directly on the floor in through-out the store and stock room areas, rather than 6 inches off of the floor as required. Observed small Pepsi cooler leaking from the bottom on to the floor in the customer aisles at the registers and is in need of repair. Observed too numerous to mention or count single-service, single-use articles stored in store area and stock area stored directly on the floor, and not 6 inches above the floor. Waste receptacle in the employee men’s room is overflowing onto the floor and in need of emptying. Women’s toilet room is not provided with a covered waste receptacle for sanitary napkins. Toilet rooms are not completely enclosed with a tight fitting door. Stall door is off of the hinges and leaning against toilet. Observed rear emergency exit door located in the back of the store of the food facility is rusted at the bottom causing a gap and does not protect against the entry of insects, rodents, and other animals. Paper towel dispenser empty at the hand-wash sink in the employee area. Corrected on sight.

Britton Coffee (Mcallister Hotel/downstairs), 11 York St. Hanover, May 17. Pass. No violations.

Greystone Brew House, 303 Golf Club Ave. Dillsburg, May 17. Pass. Burgers was held at 97°F, in the hot holding area, rather than 135°F or above as required. Case was turned on but was not hot. Corrected. Knives in the kitchen area stored on magnetic racks with dried food debris and or food residue. Corrected. Baking equipment not being cleaned every 24 hours (dough cutter and bread cutter). Personal food items (cookies, protein power, Cheez-Its) where store above food prep area in the bakery area. Rafters in the baking area are dust and dirty and need to be cleaned. Ceiling tiles in the food prep area observed with excessive food splatter and need to be cleaned. Sanitizer wipes stored in the back of the bakery area was above food packaging (Kids Cups), cleaners observed stored on the same shelf in the baking area next to pretzel salt. Corrected. Employee medication stored on top shelf above a food prep area. Corrected.

J&B’s Grill, 14 N. Main St. Shrewsbury, May 17. Pass. Food employees observed in kitchen area, not wearing proper hair restraints, such as nets, hats, or beard covers. corrected. Meatballs and sauce food, which was cooled, was not reheated to 165°F within two hours, for hot holding. Corrected. Food was properly heated. Several tongs and utensils, a food contact surface, were observed to have food residue and was not clean to sight and touch. Corrected.

China III, 340 N. Main St. York New Salem, May 16. Pass. Refrigerated ready to eat, time/temperature control for safety food prepared in the food facility- sauces, cooked chicken, cooked noodles and other foods and held for more than 24 hours, located in the walk-in and refrigerator unit , is not consistently being date marked. Corrected. Manual warewashing equipment observed with food residue and not cleaned before use, and frequently throughout the day. Observed food employee using manual warewashing sink for food preparation without sanitizing before use. Wash compartment of the manual warewash equipment contained dirty wash water. The water temperature of the wash compartment of the manual warewashing equipment was 54°F, rather than not less than 110°F, as required. Observed no chemical sanitizer being used on equipment and utensils after washing and rinsing them, and prior to use. Corrected.

Rita’s Italian Ice, 617 Lombard Rd. Red Lion, May 16. Pass. Interior floor and sides of freezer unit holding to go size containers of Rita’s ice had build-up food debris and is in need of cleaning. Observed single-service, single-use articles- lids and bowls stored in back shelving area directly on the floor, and not 6 inches above the floor. Front door to the outside, located at the entrance area of the food facility is being propped open. Corrected. Facility will keep door closed or install a screen.

Save A Lot, 1045 Mount Rose Ave. York, May 16. Pass. Top underside of the produce cold case with white matter and needs to be cleaned. Wall coving in between bathrooms are damaged cracked, roughen and need to be repaired. Flooring in the walk-in Milk cooler has two spots where concrete is exposed and floor ins rough and needs to be repaired. Meat Room with area where tile and concrete meat, tile is broken and flooring is not an easy transition. Flooring is slated to be repaired. The floor/wall juncture in meat walk-in inside the door to the left area is not coved and closed to 1/32 inch. Flooring in the walk-in freezer under storage racks with accumulation of food debris, paper and plastic.

Sleep Inn and Mainstay Suites, 1401 Toronita St. York, May 16. Pass. Observed consumer self-service display of apples without tongs/dispensing utensil or a sign stating apples should be washed before consumption. Time in lieu of temperature being used in the food facility to control ready to eat potentially hazardous foods breakfast potatoes without written procedures or documentation to verify disposition of food. Foods provided from 6:00 a.m. – 9:00 a.m. potatoes temperature was 127°F, hot holding unit is in need of repair. Provided training related to using time in lieu of temperature for holding temperature control for safety foods. Corrected. Non-food contact surfaces of the refrigerated juice dispenser are not cleaned at a frequency to preclude accumulation of dirt and soil. Customer self-service single-use tableware is not displayed in a manner so that only the handles are touched by customers. Paper towel dispenser empty at the handwash sink in the kitchen area. Corrected.

Dairyland Family Restaurant, 5 Dairyland Square, Red Lion, May 15. Pass. Scoop being stored in the ice machine with handle touching and/or buried beneath the ice. Corrected. Observed wet wiping cloths in facility not being stored in sanitizer solution. Refrigerated ready to eat, time/temperature control for safety food – cooked noodles, sliced tomatoes, cut lettuce and other foods prepared in the food facility and held for more than 24 hours, located in the walk-in and refrigerator units, is not being date marked. Corrected. Commercially processed refrigerated, ready to eat, time/temperature control for safety food lunch meat, sausage, pie filling, applesauce and other foods, located in the walk-in and refrigerator units, and held more than 24 hours, is not being marked with the date it was opened. Corrected.

Jino Sushi, 890 Country Club Rd. Dallastown, May 15. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Food Employee observed donning single use gloves without a prior hand wash. Food employees observed in kitchen area, without proper restraining of long hair. Observed 1 container of fresh strawberries above 3 bay sink that were moldy and adulterated. Corrected. facility discarded. Food utensils observed stored in a container of water which is not maintained at 135°F. Observed bags of sugar, and containers of oil food stored directly on the floor in basement area, rather than 6 inches off of the floor as required. Exposed food preparation observed in dark room in basement area under very dirty conditions. Food was subject to potential contamination from large amounts of cobwebs and dust overhead. Ceiling was open to rafters except for a few area that had been covered by plastic that was torn and frayed. Facility will immediately stop food production in this area until area has been cleaned. Beef roast observed thawing at room temperature on the food preparation counter, which is not an approved thawing method. Corrected. Hard boiled eggs cooked today, were being held at room temperature to cool and no process was in place to monitor cooling from 135°F to 70°F in 2 hours and/or from 135°F to 41°F within 6 hours, after preparation. Refrigerated ready to eat, time/temperature control for safety foods- shrimp, chicken, noodles, sauces and other foods prepared in the food facility and held for more than 24 hours, located in all cold holding units, is not being date marked. Time in lieu of temperature being used in the food facility to control ready to eat potentially hazardous sushi rice without written procedures or documentation to verify disposition of food. Food Facility did not obtain a variance from the Department before processing Kimchi, which is a specialized processing method. Not enough drainboards, utensil racks, or tables to allow for storage of soiled and/or cleaned items before and after cleaning. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Observed food employee using manual warewashing sink for food preparation/thawing without sanitizing before use. Observed no chemical sanitizer being used on equipment and utensils after washing and rinsing them, and prior to use. Food employee observed storing clean food equipment while wet, and not allowing time for draining and/or air-drying. Cleaned and sanitized utensils, are stored hanging from equipment doors with food contact surfaces/food or lip area exposed to hand contact when retrieved by food employees. The handwash sink in the kitchen prep area was blocked by carts full of dirty dishes and not accessible at all times for employee use. Waste water from facility washing machine is being disposed of through prep sink in the facility basement area where owner says dishes and equipment are washed. Corrected. Facility will immediately stop using this sink for any food preparation or warewashing and sink will only be used as a utility sink until plumbing can be corrected. Soap was not available at the handwash sink in the kitchen area. Corrected. Paper towel dispenser empty at the handwash sink in the kitchen area. Corrected. Observed employee toothpaste and toothbrush stored above 3 bay sink and on same shelf as clean serving bowls. Corrected.

Rutter’s #30, 2 Broad St. New Freedom, May 15. Pass. Observed fan guards in the walk-in cooler with an accumulation of static dust and ice. Corrected. Facility had already placed a work order with maintenance.

Bravo Supermarket, 1200 W. Market St. York, May 14. Pass. Pork in the walk-in meat cooler stored in a buggy under condenser unit is getting wet from condenser dripping water. Food Facility has a certified person, but was not on duty during hours of operation and inspection. Facility needs to have a certified person on duty during all hours of operation. Dock door located in the receiving area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals. Observed green dish soap in a plastic container with no label. Corrected.

Dragon China Kitchen, 913 Broad St. Extension, Delta, May 14. Pass. food ingredient storage containers containing allergen items such as cashews and sesame seeds, in the food prep area, is not labeled with the common name of the food. Corrected. Refrigerated ready to eat, time/temperature control for safety food – cooked noodles, coconut sauce, hard boiled eggs, sauces and many other foods prepared in the food facility and held for more than 24 hours, located in walk-in and refrigerator, is not being date marked. Corrected. Observed no chemical sanitizer being used on equipment and utensils after washing and rinsing them, and prior to use. Corrected. Old unused equipment, utensils and empty storage containers stored in all areas, should be removed from food facility.

Peach Bottom Inn, 6085 Delta Rd. Delta, May 14. Pass. Observed 4 severely dented canned items on can racks intended for use or sale in the food facility. Corrected. Observed wet wiping cloths throughout food prep area, not being stored in sanitizer solution. Corrected. Chicken and pork chops were observed thawing in standing water at the prep sink, which is not an approved thawing method. Corrected. Refrigerated ready to eat, time/temperature control for safety food -mashed potatoes, cooked vegetables, sauce, tiramisu prepared in the food facility and held for more than 24 hours, located in the walk-in and refrigerator, is not being date marked. Corrected. Fan blades and guard above food preparation table area of the food facility is dusty, and in need of cleaning. Hoods above stove area of the food facility are extremely greasy and in need of cleaning.

Prince Athletic Association, 600 S. Richland Ave. York, May 14. Pass. Storage shelves in the walk-in cooler observed rusted and needs to be replaced or repaired. Ceiling in the walk-in cooler with peeling paint and needs to be repaired. Interior and exterior of microwave in need of cleaning. Metal rack holding cutting boards is dusty and in need of cleaning. Flooring and metal wall behind cooking equipment grease and dusty and in need of cleaning.

Bruster’s Real Ice Cream, 1035 George St. Hanover, Opening, May 13. Pass. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. (QUAT). Food facility does not employ a certified food employee as required. Facility must enroll a food employee in a certified food safety certification course with in 14 business days).

Christ Lutheran Child Development Center, 126 W. Main St. Dallastown, May 13. Pass. No violations.

Eisenhower Diner, 375 Eisenhower Dr. Hanover, May 13. Pass. Numerous food products in walk stored open with no covering. (CORRECTED). Chicken Breast observed thawing in standing water in a large 5 Gallon Bucket of water sink, which is not an approved thawing method. (CORRECTED DISCUSSED 4 WAYS TO THAW PROPERLY). Loose or broken door hinges observed on walk in freezer unit. Observed significant amount of ice condensation inside of the walk-in freezer and is not in good repair. The quaternary ammonia concentration in the sanitizing solution bucket sink was 0ppm, rather than 200-400ppm as stated on the manufacturers use directions. Microwave unit located on cook line a food contact surface, was observed to have food residue and was not clean to sight and touch. (CORRECTED). Exit door located in the kitchen area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals. Observed flooring throughout facility under and behind and in hard-to-reach areas extremely dirty with food and debris accumulation.

Philly Pretzel Factory, 2058 S. Queen St. York, May 13. Pass. Food employees observed in kitchen area, not wearing proper hair restraints, such as nets, hats. Corrected. Wooden proofing boards in the facility are raw unsealed, absorbent and flaking. Severely worn boards need to be replaced. Tongs, Scissors and can opener blade a food contact surface, was observed to have food residue and was not clean to sight and touch. Corrected. All non food contact surfaces, counter tops, cabinets, shelving, interior top of the slushy machine lids. Floor in the bakery area is made of tile and is cracked/roughened and is not a smooth, easily cleanable surface. Coving (baseboard) by the three-compartment sink is unsealed from the wall and needs to be reattached. The handwash sink in the dough and baking area does not have single use towels, continuous towels, or air drying device. Corrected. Flooring area of the food facility is extremely dirty, dusty, and in need of cleaning. Ceiling vents and wall vents in the facility with excessive dust build-up and in need of cleaning.
