The Pennsylvania Department of Agriculture, 866-366-3723, uses a risk-based inspection reporting process for restaurants and other food handlers.

Shay’s, 35 S. 8th St. Lebanon, April 14. Pass. Observed wet wiping cloths not being stored in sanitizer solution. Clean food equipment is on a painted shelf. The employee was not aware of test strips to check sanitizer concentration. No trash can by the handwashing sink and towels were not on the dispenser. One electrical outlet is missing the cover. One spray bottle was not labeled.

Berry Unique LLC, 35 S. 8th St. Lebanon, April 12. Pass. A painted wood surface is being used to store clean food equipment. Wood cannot be properly cleaned.

Schaylor Brewing Company, 33 S. 8th St. Lebanon, April 12. Pass. Paper towel dispensers were empty at every handwash sink in the facility. Note: the person in charge stated that towels are being brought from their other location ASAP and that there was an error with the towel inventory.

Latin House Restaurant Rd, 433 N. 9th St. Lebanon, April 11. Pass. A few cracks were observed in a plastic food container lid. The shelves in one reach-in refrigerator are quite rusted; they can no longer be properly cleaned and present a risk of contamination due to rust and/or chipped paint falling into food below. Facility has a pest control program; however, the program is not being properly monitored at an appropriate frequency.

Mancino’s Pizza, 2294 Lebanon Valley Mall, Lebanon, Complaint, April 11. Pass. No violations.

American House, 25 S. 9th St. Lebanon, April 10. Pass. One container of food lids and a drawer of food utensils, both food contact surfaces, were observed to have a small amount of food residue and were not clean to sight and touch.

Guild, 170 N. 9th St. Lebanon, April 10. Pass. The interior of the microwave, a food contact surface, was observed to have food residue and was not clean to sight and touch. Cleaned. The interior of the ice machine, a food contact surface, was observed to have a slight amount of dark residue and was not clean to sight and touch.

Mark’s Pizza LLC, 748 Locust St. Lebanon, Follow-up, April 10. Pass. Observed a plastic container that had originally a food ingredient container being used for storing clean utensils; however, the plastic container was badly stained and can no longer be properly cleaned.

Robin’s Ice Mobile Unit, 545 Lehman St. Lebanon, April 9. Pass. No violations.
