The Pennsylvania Department of Agriculture, 866-366-3723, uses a risk-based inspection reporting process for restaurants and other food handlers.

Mangia Mangia Italian Grill, 2981 Elizabethtown Rd. Hershey, April 5. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed several raw meats stored above ready to eat foods in two door upright freezer next to the walk-in cooler with potential for cross contamination. Observed raw salmon and raw pork stored above ready to eat foods in the walk-in cooler with potential for cross-contamination. Observed bag of stuffed shells stored directly on top of raw pork thawing in pan in the walk-in cooler. Bag of stuffed shells were thrown away by person in charge due to cross-contamination. Observed walnuts mixed in with red pepper spice in containers in the cooking area. Red pepper was thrown away by person in charge and walnuts and spices were separated to prevent contamination. Observed package of raw bacon thawing on shelf above stove in the cooking area where food was actively being cooked with potential for cross-contamination. Corrected on site. Observed multiple food ingredient bottles/containers (walnuts, cashews, spices), in the cooking area, not labeled with the common name of the food. Observed multiple in-use knives stored between edges of prep table and countertop behind cooking line, an area not easily cleanable & sanitized. Observed multiple tongs in the cooking area stored at a height which makes them not accessible to not touching the food contact end of the utensils, causing contamination. Corrected on site. Observed single service, single-use containers stored in bins of sugar and salt without handle above the top of the food and the container. Corrected on site. Observed two bus tubs of packaged squid thawing at room temperature on the floor next to the walk-in cooler, which is not an approved thawing method. Corrected on site. Observed numerous plastic containers broken/cracked stored on racks across from three compartment sink and are no longer durable under normal use in the food facility. Corrected on site. Loose/deteriorated rubber door gaskets observed on both bain-marie refrigeration units in the pizza making area and on the walk-in cooler and are in need of replacement. Observed numerous deeply scored cutting boards not resurfaced or discarded as required. Observed the following on food contact surfaces: Two containers storing steak knives in the wait staff service area with accumulation of old food debris and crumbs are not being cleaned at a frequency, corrected on site; Plastic bin storing paper goods (straws, cups…) in the wait staff service area with accumulation of old food debris and is not being cleaned at a frequency, corrected on site; Accumulation of dirt/food residue on inside of soda gun nozzle in the wait-staff service area and is not being cleaned at a frequency, corrected on site; Manual can opener blade with accumulation of old food residue/dirt and is not being cleaned at a frequency, corrected on site; Bus tub storing variety of lids on racks across from three compartment sink with accumulation of food debris/crumbs and is not being cleaned at a frequency, corrected on site; Ice machine with accumulation of pink slime on interior surfaces, corrected on site. Observed the following on non-food contact surfaces: Static dust accumulation on vent above pizza oven and is in need of cleaning; Spray nozzle for sink next to mechanical dishwasher with accumulation of old food debris/dirt and was in need of cleaning, corrected on site; PVC piping from underneath sink to food debris trap below mechanical dishwasher with accumulation of pink slime/dirt around the end where water is dispensed and is not cleaned at a frequency; Shelving storing cooking equipment/containers throughout the food facility have accumulation of food debris and are in need of cleaning; Bottom shelf of two door upright freezer next to walk-in cooler with large accumulation of food debris and is in need of cleaning. Observed clean stainless steel and plastic containers on storage racks across from three compartment sink, stored wet in a manner that does not allow for draining and/or air drying (wet nesting). Soap was not available at the handwash sink at the salad prep station. Potential rodent harborage areas inside the food facility observed in the dry storage area due to too many to count rodent-like droppings underneath dry storage racks and on pallet beside it.

The Pinchos Spot TFS3, 1106 Herr St. Harrisburg, Opening, April 4. Pass. No violations.

Dunkin Donuts, 4612 Route 209, Elizabethville, April 3. Pass. Observed a Y-shutoff valve downstream from the atmospheric vacuum breaker. No shutoffs are allowed beyond the AVB with out a bleeder valve. REPEAT VIOLATION.

Red Robin #327, 621 Park Ave. Hershey, April 3. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed bottles of teriyaki sauce stored on shelves and not refrigerated at 41°F or below after opening as required/stated by original packaging. Interior surfaces of all microwaves in the kitchen area are deteriorating and are in need of good repair/replacement. Observed the following on food contact surfaces: Old food residue on underside of refrigeration drawers in burger assembly area and are in need of cleaning; Old food debris/crumbs in containers storing plastic sauce cups/lids on the expo line and are not being cleaned at a frequency, corrected on site; Microwaves throughout the kitchen area with accumulation of old food debris and are not clean to sight and touch; Food service trays and the area where they are stored have accumulation of old food debris and are in need of cleaning; Accumulation of old food debris/crumbs in utensil storage containers in wait staff service area and is not being cleaned at a frequency, corrected on site; Rubber holders for all soda dispensers behind the bar have buildup of slimy food residue and are not being cleaned at a frequency. Observed the following on non-food contact surfaces: Large static dust accumulation on electronic equipment in the cooking area and is in need of cleaning; Wall behind bain-marie unit and underside of shelf above it in the burger assembly area have accumulation of old food debris and is in need of cleaning; Floor drains throughout the facility have accumulation of dust, dirt, food debris and are in need of cleaning. Observed clean, stainless-steel containers on racks across from three compartment sink stored wet in a manner that does not allow for draining and/or air drying (wet nesting). All areas of the food facility are extremely dirty, dusty, and in need of cleaning.

Reeds Service Station, 700 Market St. Millersburg, Follow-up, April 3. Pass. Prepackaged foods, such as cocktail sauce, crab dip, Macaroni & Potato Salads, pumpkin roll, apple crisp, coleslaw, deviled egg trays, cherry cheesecake delight, sweet Lebanon cream cheese rolls, sandwiches and subs are not labeled properly with the ingredient statement, net weight, distributed by statement and/or nutritional facts. REPEAT VIOLATION.

Courtyard by Marriott, 515 E. Chocolate Ave. Hershey, April 2. Pass. Observed manual can opener blade, a food contact surface, stored with accumulation of old food debris and is not being cleaned at a frequency. Corrected on site. Observed the following on non-food contact surfaces: Underside of Starbucks espresso machine behind the bar with accumulation of old, sticky food residue around the dispenser and is not being cleaned at a frequency; Underside of Starbucks coffee dispenser near the front desk with accumulation of old, sticky food residue around nozzles and is not being cleaned at a frequency; Ice build-up around the condenser and on shelf below in the walk-in freezer and is in need of removal.

East Hanover Elementary School, 2673 Sandbeach Rd. PO Box 3210, Grantville, April 2. Pass. Observed multiple deeply scored cutting boards in the storage room not resurfaced or discarded as required.

Middle Paxton Elementary School, 931 Peters Mountain Rd. Dauphin, April 2. Pass. Observed 4 rubber spatulas, a food contact surface, in drawer of prep table by the walk-in cooler, with old food/grease residue and were in need of cleaning. Corrected on site.

Chipotle, 1198 Mae St. Hummelstown, April 1. Pass. Observed male employees not wearing hair restraints, such as nets or hats. Observed all handles on all refrigeration/freezing units with accumulation of food debris and not clean to sight or touch.

Days Inn Hershey, 350 W. Chocolate Ave. Hershey, April 1. Pass. Observed accumulation of old sticky food residue on top surface of juice dispenser around nozzles and were not being cleaned at a frequency. Corrected on site.

El Rodeo, 1077 W. Governor Rd. Hershey, April 1. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed 12 pans of stuffed peppers in the walk-in freezer in the basement prep area stored open with no covering. Corrected on site. Observed raw eggs stored above ready to eat lettuce in the basement prep walk-in cooler with potential for cross-contamination. Corrected on site. Observed single service, single-use containers without handles stored in spice containers in the basement prep and kitchen area. Corrected on site. Observed wet wiping cloth on cutting board behind the cook line not being stored in sanitizer solution. Observed loose/deteriorated rubber door gaskets on walk-in coolers in the take-out area and basement prep area. Observed multiple deeply scored cutting boards not resurfaced or discarded as required in the basement prep area. Observed the following on food contact surfaces in the basement prep area: Manual can opener blade with accumulation of old food residue and is not being cleaned at a frequency; Multiple dirty knives stored on magnetic knife holder above prep table, corrected on site; Manual slicer with heavy accumulation of old food residue/debris and is not being cleaned at a frequency, corrected on site; Ice machine with accumulation of pink/black mold like residue on interior surfaces and is in need of cleaning, corrected on site. Observed numerous clean dish racks stored on the floor in the ware washing area. Corrected on site. Observed cardboard, which is not a smooth, easily cleanable surface, being used as shelf coverings on dry storage racks in the basement prep area. Soap was not available at the handwash sink behind the bar. Paper towel dispenser empty at the handwash sink behind the bar. Observed old unused equipment stored throughout the entire basement area and should be removed from food facility.

Pizza Hut, 675 Main St. Hummelstown, April 1. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed male employees without beard restraints. Some, not all, food ingredient storage containers, bottles, shakers, in the entire facility, are not labeled with the common name of the food. Two food dispensing utensils in mozzarella shredded cheese in the pizza station Randell baine marie observed stored in the food and not with handle above the top of the food and the container. Observed numerous boxes of food stored directly on the floor in walk-in freezer area, rather than 6 inches off of the floor as required. Commercially processed (sausage crumble and diced ham) refrigerated, ready to eat, time/temperature control for safety food, located in the walk-in cooler, and held more than 24 hours, is not being marked with the date it was opened. Observed a large puddle of water at the entrance to the walk-in freezer. Manager stated that it comes up through the floor and they have no known source. Observed numerous utensils, lids, containers, assorted equipment, located in kitchen area, are not in good repair or condition and must be discarded or repair (if possbile). Observed the following on food contact surfaces and was not clean to sight or touch: Observed a greasy green rolling rack stored inside of the Traulsen two door warmer. Dish racks for running dishes through the dish-machine with food residue and in need of cleaning or replacement. Seven green rolling racks with food contact equipment and to go food containers in the kitchen area, with heavy accumulation of dust, grease and hair and coming in contact with food contact surfaces. Green shelves stored above and beside food contact/prep areas with heavy accumulation of static dust, grease and hair with the possibility to contaminate food being prepared below or beside. A large tub of a cinnamon sugar mixture with exterior of tub with heavy accumulation of dried food debris. Also, interior scoop is not being removed daily for cleaning. Four gray containers on the top shelf of a prep table at the back of the kitchen, with lids stored inside, observed with food debris in the containers and touching food contact lids. Several stainless bins in the Randell Baine Marie Pizza Station not being emptied daily into clean containers. The facility was closed on Easter Sunday and bins were noticeable with dried food residue. The shelf with the order ticket machine with grease and food debris on the shelf coming in contact with black souffle cups, silver to go pans and a set of tongs. Solwave microwave interior and exterior with food debris accumulation. Observed the following on non-food contact surfaces: Black mold-like substance on all of the walls surround the three bay sink and dish machine and in need of cleaning. Dish machine with food accumulation on the exterior front, sides and top with the possibility to contaminate dishes being cleaned inside. Green rolling rack next to the three bay dish machine with heavy accumulation of grease, dust and food debris. Exteriors of all cooling, freezing, and warming units (including the glass walk-in door) with accumulation of dried food debris on the handles and exterior doors and in need of cleaning. Ticket/order machine with dried food building up which could cause cross contamination even on a gloved hand when reaching for order tickets. Interior bottom of Randell baine marie pizza unit with accumulation of food debris and not clean to sight or touch. Dead bugs, and accumulation of static dust in/on light fixtures. Accumulation of static dust on ceiling tiles, vents, equipment tops. All in need of cleaning to reduce contamination. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Observed single-service, single-use articles(a large volume of pizza boxes, three cases of take-out containers) stored in dry stock area directly on the floor, and not 6 inches above the floor. All hand-wash sinks with accumulation of slime and dirt and in need of cleaning. Exit door located in the back area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals. Exit door to the outside, located in the back area of the food facility is not self-closing and/or is being propped open. Old unused equipment stored in entire facility, should be removed from food facility.
