Beef was held at 125°F, chicken was held at 114°F in the front counter area, rather than 135°F or above as required. Observed cold holding equipment, in cookline area, with an accumulation of encrusted food residue, and debris on non-food contact surfaces. The handwash sink for employees located in the dish area, is not convenient and easily accessible, as evidenced by the observed lack of proper handwashing by employees. Corrected.
