Hardee’s #1383176, 820 E. Main St. Dallastown, March 15. Pass. Roast beef that was being portioned was left out at room temperature and was held at 65°F, rather than 41°F or below as required. Corrected. Food was within 4 hour time frame and was placed back in walk-in cooler until cooled to below 41°F. Commercially processed refrigerated, ready to eat, time/temperature control for safety food; sausage and ham mixture, shredded cheese, located in the refrigerator unit, and held more than 24 hours, is not being marked with the date it was opened. Corrected. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. specifically the floors and walls underneath the 3 compartment sink, and the exhaust hood above fryers and the walls of the Blodgett oven. The air vents above the ice machine and above the clean side of the warewashing area have a large buildup of static dust and a black mold-like residue. Ceiling tiles throughout the facility are stained and need replaced.

Corrected; items were date marked/provided training. A working container of cleaner Lysol spray and Stainless Steel Cleaner was stored on the same shelf with equipment in the three basin sink area. Corrected. Observed insecticides (Raid flying insect killer) in the three basin sink area, not labeled by the manufacturer as approved for use in a food facility. Corrected. Lubricants (WD40)being used on mixer equipment are not approved for use on food equipment. Corrected.

Pizza Town USA, 50 W. Maple St. Wrightsville, March 15. Pass. Time in lieu of temperature being used in the food facility to control ready to eat potentially hazardous foods (pizza) without written procedures or documentation to verify disposition of food. Corrected. Slicer (not used today), a food contact surface, was observed to have food residue and was not clean to sight and touch. Corrected.

Roburrito’s, 397 W. Main St. Dallastown, March 15. Pass. Commercially processed refrigerated, ready to eat, time/temperature control for safety chicken, beef and pork, located in the refrigerator, and held more than 24 hours, is not being marked with the date it was opened. Corrected. Ice chute of soda machine, a food contact surface, was observed to have green and black mold like residue and was not clean to sight and touch. Corrected. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Observed overhead vents, areas around prep table and walls behind refrigerators in the back room, to have food splatters and build up of old food residue and are in need of cleaning.

Denny’s Restaurant #9582, 199 Eisenhower Dr. Hanover, March 14. Pass. Food Employee observed changing tasks that may have contaminated hands without a proper handwash in-between. (DISHWASHER OBSERVED MOVING FROM DIRTY TO CLEAN NO HANDWASH CORRECTED). Observed ceiling and deflector plate of large bulk ice machine a food contact surface, was observed to have blackish residue and was not clean to sight and touch. (CORRECTED). Observed dishwasher touching clean utensils on food contact surfaces. Food employee observed storing clean food equipment while wet, and not allowing time for draining and/or air-drying. Observed 8 Gallon stainless steal pot filled with grease from prior day and was not properly disposed of. (CORRECTED). Floors and drains located throughout kitchen area of the food facility is extremely dirty, dusty, and in need of cleaning. Ceiling vents throughout facility have accumulation of dust and dirt and need to be cleaned.

Dunkin Donuts, 2210 Old Trail Rd. Goldsboro, March 14. Pass. The Food Facility Operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation. An open employee’s beverage container was observed in toppings/sandwich area, a food preparation area. Beverage stirrer spoons in the espresso maker area observed stored in a container of water which is not maintained at 135°F. Food Facility has a certified food employee, but a certified food employee was not available during all hours of operation. Plumbing system not maintained in good repair; observed three basin sink leaking at the center basin waste water pipe.

Longhorn Steakhouse Restaurant #5319, 2901 Concord Rd. York, March 14. Pass. Raw chicken was stored above raw pork inside the walk-in cooler. Corrected. Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in cold holding units on the cooks line. Fan blade covers are not in place in three of four microwave units on the cooks line. Observed cooking equipment with an accumulation of dust, dirt, food residue, debris on non-food contact surfaces. Non-food contact surfaces throughout kitchen are not cleaned at a frequency to preclude accumulation of dirt and soil. The handwash sink located in the bar area does not have water. Corrected -Hand washing temporarily diverted to the bar prep area hand sink. Ceiling and light fixtures on the cooks line have heavy coating of static dust. Hood filtration system baffles have a heavy accumulation of grease and soil.

Royal Farms #214, 17302 Draco Rd. Stewartstown, March 14. Pass. Observed one box of raw chicken stored directly on the floor in walk-in cooler and one box of soft pretzels stored directly on the floor in the walk-in freezer, rather than 6 inches off of the floor as required. Observed employee personal items such as wallet, phone, coat and open drink in food prep areas, potentially a source of contamination of food. 6 chicken racks and a large number of food pans and several utensils stored as clean, were observed to have large build up of old food residue and were not clean to sight and touch. Corrected. Facility washed and sanitized all dirty pans and racks. Observed underside of Pepsi fountain with an accumulation of syrupy food residue, on non-food contact surfaces. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Specifically the walls and floors behind the shelving units in the walk-in and the fryer cart and areas under chicken coating equipment.

Employee was given instructions on proper handwashing and glove use. Observed food unprotected in front of a blowing fan that was dirty with static dust, and is subject to potential contamination. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Facility is using quat sanitizer but has Iodine test strips. The hot water used for sanitizing in the mechanical warewashing machine did not reach 180°F (or 165°F for stationary rack). Corrected. Facility will use 3 compartment sink for sanitizing until the mechanical warewashing is in compliance. Mechanical warewashing equipment observed not being operated according to the manufacturers data plate.

Commercially processed refrigerated, ready to eat, time/temperature control for safety food (American Cheese), located in the refrigerator, and held more than 24 hours, is not being marked with the date it was opened. There is not a manufacturers use by date on ASK Creamy Cole Slaw 5 lb. container the lid is swollen and the food requires discarding at this time. Filthy dirty sponges observed in the three basin sink area being used to clean food contact surfaces. Interior surface of chest freezer is cracked and gasket is torn. Loose or broken door gasket observed on the chest freezer. Heavy accumulation of frozen condensate observed inside the chest freezer. Manual warewashing equipment observed with heavy build up of filth and food residue and not cleaned before use, and frequently throughout the day. Upper interior of three of three microwave units, food contact surfaces, were observed to have food residue and were not clean to sight and touch. Observed fryer and flat grill non food contact surfaces with encrusted grease and soil accumulation. Observed storage equipment, in all areas, with an accumulation of dust, dirt, food residue, debris on non-food contact surfaces. Food temperature measuring device is not being cleaned before use or storage. Six thermometers are filthy and lens’ are so grease/dirt laden that the dials cannot be read. Cooking/frying/baking equipment not being cleaned every 24 hours as evidenced by a heavy accumulation of food debris and dirt. Non-food contact surfaces throughout the food service and food storage areas are not cleaned at a frequency to preclude heavy accumulation of dirt and soil. Entire interior of cold holding and cooking equipment units are extremely filthy and in need of cleaning. Observed soiled linens stored in and on the three basin sink area which may contaminate food, utensils, or equipment. Cleaned and sanitized utensils located in customer service area, are stored with food contact surfaces in direct contact with surfaces that are extremely soiled. Single use paper wrapped straws are splattered with pink/red food residue and must be discarded at this time. A Restricted use pesticide (Raid wasp and hornet spray) is being applied in the food facility by a non-certified applicator. Observed chemicals (hand sanitizer) and pesticides (Raid Wasp and Hornet spray) stored directly to the right of the three basin sink area on shelving above and in direct contact with spices, utensils, and equipment.

Observed unauthorized persons in food preparation back room and food preparation customer service areas. The Person in Charge failed to control access to food facility by unauthorized persons. Food Employee observed changing tasks that may have contaminated hands without a proper handwash in-between. Food Employee observed donning single use gloves without a prior hand wash. Food employees observed in customer service/food preparation area, not wearing proper hair restraints, such as nets, hats, or beard covers. Facility does not have written procedures for employees to follow when responding to an event involving vomitus or fecal matter discharge onto surfaces within the facility.
