The Pennsylvania Department of Agriculture, 866-366-3723, uses a risk-based inspection reporting process for restaurants and other food handlers.

8th St. US Gas, 742 Walnut St. Lebanon, March 15. Pass. Hot chicken produced at another facility was held at 111°F, in the display area, rather than 135°F or above as required. The chicken was removed from the sales area. Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in hot-holding equipment where chicken was located. Thermometers for ensuring proper temperatures of food are not available in this facility. Facility is not using the 3-compartment sink for washing food equipment as evidenced by one compartment being completely blocked by a large container in it. Excessive trash on the ground behind the dumpster. Corrected. The lids were not closed.

 

Mark’s Pizza LLC, 748 Locust St. Lebanon, March 15. Fail. When asked why there was no soap at the handwashing sink, the employee responded that the owner does not want them washing hands at the handwashing sink and that they are supposed to wash hands at the 3-compartment sink in the rear prep area. The owner arrived later during the inspection and confirmed this fact. He stated that he is concerned about splatter if employees wash at the handwashing sink. Owner was informed that if splatter is a concern, then barriers shall be installed to prevent splatter. The rear prep/warewashing area does not have a handwashing sink. Plastic food containers and rubber spatulas are deteriorated. Plastic food ingredient containers are being re-used in the facility; these containers are badly stained and cannot be properly cleaned. A container of mixer parts and a container of food utensils, both food contact surfaces, were observed to have food residue and were not clean to sight and touch. The food facility does not have a handwash sink located in the rear food prep/warewashing area. Soap was not available at the handwash sink in the front prep/service area. Potential rodent harborage areas inside the food facility observed in the food facility as two dead mice were found on a glue trap in the basement food storage area and one dead mouse was found in the rear food prep/warewashing area. A spray bottle of degreaser and a container of bleach, both working containers in the rear warewashing area, used for storing chemicals/cleaners taken from bulk supplies, were not marked with the common name of the chemical.

Little Caesars, 720 Quentin Rd. Lebanon, Complaint, March 14. Pass. Two items noted on the previous inspection from 2/16/2024, have not been fully corrected: Cardboard is being used to cover the lower level of several prep tables. Cardboard cannot be properly cleaned; if cardboard is going to be used to cover areas that accumulate grease or debris, the cardboard should be changed frequently as it becomes soiled. The lower level of one stainless prep table has a plastic covering which is very deteriorated and cannot be properly cleaned. The plastic covering shall be removed and the table shall be properly cleaned.

Mays Wok Chinese Cuisine Inc., 181 S. Lancaster St. Jonestown, March 14. Pass. Exit door located in the rear of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals.

Navy Club of Lebanon #91, 1501 Lehman S.t Lebanon, March 14. Pass. Beef gravy and other foods, which were cooled from previous service, was only reheated to temperatures between 90°F and 115°F for hot holding and not 165°F for 15 seconds as required. Food is being reheated in equipment that is intended for hot-holding and not for cooking. Food was held at 90°F to 115°F, in the hot holding unit, rather than 135°F or above as required. The exterior surface of the soup kettle warmer is not clean from previous service on a previous day. Soap and towels were not available at the handwash sink in the prep area.

Save A Lot Food Stores #683, 611 12th St. Lebanon, March 14. Pass. The area within the enclosure is not being properly maintained; there was a very large amount of garbage and trash on the ground around the dumpster. The trash can at the front entrance is not being emptied frequently enough as evidenced by the fact that it was overflowing. Toilet tissue is not on the dispenser in the women’s toilet room. The frequency of cleaning in the rear storage room is inadequate as evidenced by a significant amount of trash and debris scattered around the perimeter of the room. The frequency of cleaning the fan covers in the dairy walk-in cooler is inadequate.

Cookies Cupcakes and More LLC, 157 E. Market St. Jonestown, March 13. Pass. Food facility has an employee that has taken food safety training program; however, the food safety program was not an ANSI/CFP accredited Certified Food Manager program. PRIOR VIOLATION 07/28/2023.

Himalayan Groceries Palmyra LLC, 609 W. Main St. Unit 5, Palmyra, March 13. Pass. Toilet rooms do not have a self-closing door.

BG’s Value Market, 19 Old Route 22, Jonestown, March 12. Pass. Non-food contact surfaces (fan guards) in the walk-in coolers are not cleaned at a frequency to preclude accumulation of dust.

Burger King, 8 Fisher Ave. Jonestown, March 12. Pass. Food employee observed in the food preparation area, not wearing a proper hair restraint. Non-food contact surface (floor) in the food preparation area is not cleaned at a frequency to preclude accumulation grease and old food particles.

Love’s Travel Stop #366, 22 Old Forge Rd. Jonestown, March 12. Pass. The handwash sink in the Chester’s chicken food preparation area was blocked by a plastic container and not accessible at all times for employee use.

Maennerchor, 301 E. Mifflin St. Lebanon, March 12. Pass. Scoop being stored in the ice with handle touching the ice. Corrected by placing two small foil trays on top of the ice bin.

Smoke & Chill, 436 E. Lehman St. Lebanon, March 12. Pass. No violations.

Subway #47404, 813 Bowman St. Lebanon, March 12. Pass. Drink cups, coffee cups and lids are stored next to the handwashing sink where they are subject to splash from handwashing. The ice spout on the soft-drink fountain, a food contact surface, was observed to have a pink residue and was not clean to sight and touch. Note: This violation was observed on inspections in 2019, 2021, 2022 and 2023. A warning was included on the 2023 Inspection Report: “If it is not corrected tickets and/or citations will be issued.”.

Chipotle Mexican Grill #2902, 1451 Quentin Rd. Suite 100, Lebanon, March 11. Pass. Non-food contact surface (floor) in the warewashing area is not cleaned at a frequency to preclude accumulation of dirt and soil. Customer self-service single-use tableware is not displayed in a manner so that only the handles are touched by customers.

Holiday Inn Express & Suites, 2205 Cumberland St. Lebanon, March 11. Pass. Thermometers for ensuring proper temperatures of food are not available or readily accessible. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Food facility does not employ a certified food employee as required.
