Shay’s Bakery, 132 E .Main St. Palmyra, Change of Owner, Feb. 22. Pass. Observed consumer self-service display of bread without sneeze guards or other effective protection.

Big Lots #0494, 467 W. Penn Ave. Cleona, Feb. 21. Fail. Food facility hot water heater is not producing enough hot water to supply handwash sinks at the time of this inspection.

Justino’s Pizza, 28 E. Main St. Newmanstown, Feb. 21. Pass. Three bay sink observed with build up of food residue and not cleaned before use.

Phovina, 115 Springwood Dr. Suite 200, Lebanon, Feb. 21. Pass. Beef observed thawing at room temperature on a shelf in the kitchen, which is not an approved thawing method.

12th Street Quick Stop Food Mart, 135 S. 12th St. Lebanon, Feb. 19. Pass. No violations.

Brother’s Food Max Lebanon LLC, 940 Church St. Lebanon, Follow-up, Feb. 19. Pass. Garlic for sale in the produce section is not labeled with the name of the item or with dates. This is a repeat item. Scoop for ice machine is stored on top of the ice machine instead of being stored in a manner to prevent contamination. Several items that were made on Friday, Feb. 16 had stickers from a previous date. The fan covers in the Dairy walk-in cooler have an accumulation of dust and debris and are not being cleaned frequently enough. The interior of a cabinet used for ingredients and equipment in the kitchen has old food debris and other items that should be removed. Two containers had food debris in the bottom of them. These items were cleaned immediately. The perimeter of the produce cutting room has a significant amount of debris and items that should be removed particularly under the produce prep table.

Latin American Supermarket, 429 N. 9th St. Lebanon, Feb. 19. Fail. Deteriorated carrots with a date of Jan 25 were for sale in the produce area. Discarded. Scoop for the ice machine is stored on the machine instead of being stored in a manner to prevent contamination. Removed. Utensils that are for cleaning large food equipment are not properly identified to differentiate them from equipment used for cleaning the floor. The three fan covers on the ceiling of the meat cutting room are extremely dirty and in need of cleaning. Observed a wooden piece of furniture that was intended for changing a baby being used as a shelf for supplies in the meat area. Observed another shelf/cart that is extremely rusted. Several cardboard boxes are being used for storing ingredients and other items in the meat room/meat & deli area. Cardboard cannot be cleaned and if it is going to be used, should be changed frequently. The interior of the ice machine, a food contact surface, was observed to have brown residue and was not clean to sight and touch. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil; specifically food debris was observed under the slicer, on the floor behind the slicer prep table, on the cords for the slicer and other equipment. Paper and foil covering on the shelves of the produce section has deteriorated and cannot be properly cleaned. One glass door on the meat display cases is broken and covered with cardboard. The cove molding in deli/meat display case area has deteriorated and is falling off of the wall. The material along the floor/wall junction behind the prep table with the meat grinder is also in poor condition.

North China, 720-1 Jonestown Rd. Lebanon, Feb. 19. Pass. A Food Employee was observed washing their hands at the prep sink rather than the designated handwash sink. Observed two plastic bus tubs in poor condition. One was broken and another was deteriorated and unable to be properly cleaned. Removed from service. Deli-containers, an item that is intended for single-use, being used as scoops in meat and dry rice instead of a durable scoop with a handle. Discarded. The cold water for the handwashing sink is not functioning. This needs to be corrected immediately. The light intensity in the small room by the walk-in cooler is inadequate. There is a very small roof leak in the rear of the room by the walk-in cooler. Potential rodent harborage areas on the exterior of the building perimeter observed in the rear area due to rubbish and leaves along the building. One spray bottle, a working container used for storing chemicals/cleaners taken from bulk supplies, was not marked with the common name of the chemical.

Turkey Hill #188, 1201 Maple St. Feb. 19. Pass. Frequency of cleaning under shelves in the walk-in cooler is inadequate as evidenced by debris and spills on the floor.
