The Pennsylvania Department of Agriculture, 866-366-3723, uses a risk-based inspection reporting process for restaurants and other food handlers.

Angelo’s Pizza & Pasta, 317 Union St. Millersburg, Change of Owner, February 2. Pass. A food employee was observed touching pizza – a ready to eat food – with bare hands. Observed eggs and potatoes stored directly on the floor in walk-in cooler area, rather than 6 inches off of the floor as required. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Observed a dust accumulation on the fan guards of the walk-in cooler. Observed the handsink in the basement prep area was not draining.

Bliss Blends Cafe, 121 Towne Square, Hershey, February 2. Pass. Multiple food ingredient storage containers above the two-door cooling unit, are not labeled with the common name of the food. Loose rubber door gaskets observed on two door Traulsen cooler under bain-marie and are in need of replacement. Observed napkins stored in directly on the floor next to the upright refrigerator, and not 6 inches above the floor.

Hershey Italian Lodge, 128 Hillcrest Rd. Hershey, February 2. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. An open employee’s beverage container was observed stored on a shelf above the clean dish line in the dishwashing area. Food employees observed in cooking area, not wearing proper hair restraints, such as beard covers. Observed opened gallon of Galliker Milk with an expiration date of 1/26/24 in the walk-in cooler. Discarded by management. Observed Apple sauce date marked 1/11/24 stored in two-door refrigerator in cooking area. Discarded by management. Observed chicken wings stored in the walk-in cooler open with no covering. Observed food ingredient storage containers/bottles, in the numerous areas of the cook/prep area, not labeled with the common name of the food. Observed large cooking pots stored directly on the floor in back storage area. Observed racks holding glassware stored directly on the floor underneath table in dishwashing area. Observed wet wiping cloths in the cooking area, not being stored in sanitizer solution. Loose rubber door gaskets observed on both walk-in cooling unit doors and are in need of replacement. Observed the following on food contact surfaces: Old food residue on 3 knives stored in holder on Bain-Marie unit; Old food debris on hood above cooking area where pots and pans are stored coming in contact with contamination; Heavy accumulation of food debris underneath shelf where ready to use plates/dishes are stored for food service. Unwrapped and unprotected single use straws stored in the customer area on bar. Ceiling tiles in the back prep area are peeling/cracking and is not a smooth, easily cleanable surface. Screen door located in the back prep area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals. Paper towel dispenser empty at the handwash sink at the entrance to the kitchen. No sign or poster posted at the handwash sink in the back prep area to remind food employees to wash their hands.

Primanti Bros., 131 W. Chocolate Ave. Suite 121, Hershey, February 2. Pass. Food employees observed in food prep area, not wearing proper hair restraints such as beard covers. Assorted cooked chicken parts in the walk-in cooler stored open with no covering. Educated and corrected on site. Single use souffle cup stored in Cajun seasoning bucket and not with handle above the top of the food and the container. Thermometers for ensuring proper temperatures of food are not available or readily accessible in multiple refrigerating units. Observed numerous deeply scored cutting boards not resurfaced or discarded as required and are in need of replacement. Observed the following on food contact surfaces: Old food debris around blade of manual can opener on the large cutting board table; Old food debris around blades of French fry cutter stored on bottom shelf of food prep table near the walk-in cooler; Old food debris on automatic slicer and dough press machine in the back prep area. All were corrected on site and Person in Charge was educated. Ceiling vents and white pole by the cook line observed to have accumulation of static dust/dirt and is in need of cleaning. Observed large old food debris accumulation in drain underneath 3-bay sink and is in need of cleaning.

Yah Brew, 251 W. Chocolate Ave. Hershey, February 2. Pass. Observed case of raw eggs stored above ready to eat foods in the walk-in cooler. Observed deeply scored cutting board on bain-marie unit not resurfaced or discarded as required. The blade on the manual can opener, a food contact surface, was observed to have food residue and was not clean to sight and touch.

Chautari Restaurant, 5890 Derry St. Harrisburg, Follow-up, February 1. Pass. No violations.

Dollar Tree #7572, 3243 Paxton St. Harrisburg, February 1. Pass. Observed walk-in freezer in the back storage area, with accumulation of ice build-up on the floor.

East Hanover Elementary School, 2673 Sandbeach Rd. PO Box 3210, Grantville, February 1. Pass. Observed numerous food serving trays having large amount of discoloration and are in need of replacing at this time. Observed leak in back flow device in the dishwashing area located above stainless steel counter and is in need of repair. Observed spray nozzle used for dish washing with accumulation of dirt/food debris and is need of cleaning.

Lucky Mart Sunoco, 4101 N. Front St. Harrisburg, February 1. Pass. Observed eggs in carton stored above ready to eat foods in the open display case. Manager corrected this violation by re-locating eggs to the bottom shelf. Note: raw food are prohibited from being stored above ready to eat foods. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Several shelves in the walk-in cooler have mold growth on the shelves. Floor in retail area has 10 missing floor tiles. Manager said a contractor has been contacted to install new tiles.

Mom’s Momo and Deli, 5550 Derry St. Harrisburg, February 1. Pass. Sauces and cut vegetables in the walk-in cooler and bain marie refrigerators area stored open with no covering. Corrected. Observed food ingredient storage containers and bottles, through-out the facility, is not labeled with the common name of the food. Observed cardboard being used for shelf lining in the back storage area. Mops are not being hung to air dry.

The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. The Person in Charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions. Food employee observed in kitchen area, wearing bracelet. Food employees observed in kitchen area, not wearing proper hair restraints, such as nets or hats. Observed two boxes with raw eggs stored above of boxes onions, stored in the walk-in cooler. Uncovered food was observed around the facility in the different refrigeration units and outside of them (spices, shrimp, chopped okra, sugar…). Observed food ingredient storage containers and bottles, through-out the facility, is not labeled with the common name of the food. Observed souffle cup, no handle, stored inside of different spice and grain. Prepackaged yogurts, smoothies, and other various ‘to-go’ foods were not labeled properly with the name of product, ingredient statement, net weight, distributed by statement, nutritional facts, and/or allergen warning “Big 8” statement. Discussed with Person in Charge. Observed two broken spoon and one black container and not easily cleanable. Corrected on sight. The wash solution temperature in the mechanical warewash equipment sanitizing with chemicals was 90°F, rather than not less then 120°F, as required. It was discussed with the owner that the three bays sink should be used while repairing the equipment. Observed deeply scored cutting boards not resurfaced or discarded as required. Paper towel dispenser does not work at the handwash sink in the kitchen area. Plumbing system not maintained in good repair. Observed the faucet leaking at the hand-wash sink next to the cook area.

St. Thomas Restaurant, 5974 Linglestown Rd. Harrisburg, January 31. Pass. Food employees observed in kitchen area, not wearing proper hair restraints, such as nets or hat. During the inspection, 1 gallon milk were found expired (Jan. 25 2024). Observed the following, throughout the facility area, with an accumulation of dust, dirt, food residue, debris on non-food contact surfaces: Exterior (especially the handle areas of the units) surfaces of all refrigeration equipment.

Courtyard Marriott Harrisburg/Hershey, 725 Eisenhower Blvd. Harrisburg, January 30. Pass. During the inspection, 9 Chobani Yogurts 5.3 oz of different flavor were found expired (8 yogurts Jan. 26, 2024 and 1 yogurt Dec. 21, 2023). Corrected. Observed numerous food ingredient storage containers (Brown sugar, caramel, oil…), in the Bakery area, is not labeled with the common name of the food. Observed sticky handles on doors below soda machine and sticky soda residue around nozzles under dispensing housing and dispensing bars in self-serve area and were not clean to sight and touch. Observed deeply scored cutting boards in assembly area not resurfaced or discarded as required. Observed knives stored in plastic container behind assembly area, a food contact surface, not cleaned/washed from previous night. Corrected on site. Observed manual slicer stored on shelf under prep table, a food contact surface, with accumulation of dry food debris on the blades and not clean to sight and touch. Observed small stainless ladle and white cylinder container hanging above 3 bay sink with two spatulas inside coming in contact with dry food debris and was not clean to sight and touch. Corrected on site. Old unused equipment stored in back prep area, should be removed from food facility. Observed four knives and pair of scissors stored dirty in magnetic holder above prep table. Manager stated that equipment was not used. Educated.

Uncle Moe’s Grab N Go, 555 Walnut St. Harrisburg, Opening, January 30. Pass. No violations.

Wasabi Bistro, 1311 E. Chocolate Ave. Hershey, January 30. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed dry chow noodles stored on shelf in the entrance to the kitchen area were open with no covering. Corrected on site. Observed numerous plastic containers of cut vegetables stored in the True upright two door refrigerator open with no covering. Observed numerous food ingredient storage containers/bottles, in the cook, prep, and sushi area, not labeled with the common name of the food. Four spatulas stored in metal pan next to the griddle observed stored in a container of water and scoop stored in bucket of water in the cooking area, which were not maintained at 135°F. Single use containers without handles observed stored in numerous food containers (flours, spices, sauces) throughout the facility. Observed wet wiping cloths in the cooking area, not being stored in sanitizer solution. Loose rubber door gaskets observed on the two-door Victory cooling unit under bain-marie and upright two-door True refrigerator. Observed deep scoring on Teflon pan stored in broiler oven over the stove. Exterior of toaster oven on the shelf next to the gas stove with heavy accumulation of food and in need of cleaning. Exteriors of numerous flour and spice containers with accumulation of dry food debris throughout the facility and are need of cleaning. Observed range hood with accumulation of dripping grease and heavy buildup of static dust on exterior grids and is need of cleaning. Observed shelf under griddle on the cook line with heavy accumulation of dirt/dust/old food residue and is need of cleaning. Observed underside of shelf above the clean, ready to use, dishes having accumulation of dry food debris with possibility of contaminating dishes stored below and is need of cleaning. Observed three shelves with to-go containers, bags, etc. stored not cleaning to a frequency to prevent contamination. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Observed clean food equipment and/or utensils on shelving next to 3 bay sink and shelving next to the dish machine, stored wet in a manner that does not allow for draining and/or air drying (wet nesting). Two garage doors located in the back storage area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals. Observed hole in the floor in the back dry stock area and is broken/cracked and in need of repair. Observed Corion countertop at the handwash sink station in back kitchen area with cracks/divots in the surface and is no longer smooth and easily cleanable. Old unused equipment/utensils stored throughout the entire facility, should be removed/discarded.

Giant Food #6097, 1250 Cocoa Ave. Hershey, January 29. Pass. Observed large opening at the base of the wall by the exterior door of the walk-in cooler in the seafood department and is need of repair. Observed numerous icing tips for cake decorating stored in individual plastic shelving drawers. Drawers had food debris coming in contact with the tips and was not clean to sight and touch. Corrected on site. Observed accumulation of dirt/food/grease debris in the floor drain at the entrance to the walk-in cooler in the seafood department and is in need of cleaning. Observed top exterior of the Hobart dish machine in the bakery having heavy accumulation of food/dirt debris and was in need of cleaning.

Loose/deteriorated rubber door gaskets observed on the right door of the True two door upright freezer unit in the back storage area. Observed blue brush, stored in a cabinet with food, that is used to sweep crumbs off of the counters. Brush is not washed, rinsed and sanitized at a frequency to prevent contamination on surfaces contacted. Educated and corrected on sight. Observed a stack of yellow sandwich pull trays stored on the floor in the back storage area and not cleaned at a frequency to prevent contamination. Educated and corrected on sight. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Observed accumulation of food debris, water in cooler bottoms, sticky/tacky handles on cabinet doors, spilled dried coffee/tea stains on interior shelving of cabinets. Paper towel dispenser empty at the hand-wash sink in the back area next to the three bay sink. Corrected on sight.
