The Pennsylvania Department of Agriculture, 866-366-3723, uses a risk-based inspection reporting process for restaurants and other food handlers.

Giant Food Store #6266, 835 Bowman Ave. Lebanon, Dec. 7. Pass. Several hotel pans in the produce department were observed to have food residue and were not clean to sight and touch. The person in charge believed that the pans are not being used regularly and would be cleaned prior to being used. Two of the four covered waste receptacles for sanitary napkins in the women’s toilet room are missing the covers. Person in charge ordered four new receptacles during the inspection. One spray bottle, a working container used for storing chemicals/cleaners taken from bulk supplies, was not marked with the common name of the chemical. Corrected.

Hissho Sushi at Giant 6266, 835 Bowman St. Lebanon, Dec. 7. Pass. No violations.

Porch, 890 Tulpehocken Rd. Myerstown, Dec. 7. Pass. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil.

Quesa Dee’as, 708 Cumberland St. Lebanon, Follow-up, Dec. 6. Pass. Chicken, beef and fish were all stored in one container. Person in charge corrected. Several items in the bottom of the bain marie were stored without being covered. A container of pork in the reach-in refrigerator was dated 11/27 which was ten days prior to the date of this inspection. Item was discarded. One container used for storing chicken was cracked. Discarded. Several food contact surfaces, were observed to have food residue and were not clean to sight and touch; specifically, there was debris in the bottom of the clean silverware caddy and the can opener was very dirty. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Specific examples are the sides of the bain marie and the edges of the sink.

Dunkin Donuts, 1950 Quentin Rd. Lebanon, Dec. 5. Pass. Food employee observed in the food preparation area, not wearing a proper hair restraint.

Fifth Avenue Beverage Company, 700 S. 5th Ave. Lebanon, Dec. 5. Pass. Food facility is using an approved non-public water system, but does not have current laboratory testing results for water potability.

Grocery Outlet 739, 1301 Quentin Rd. Suite 400, Lebanon, Dec. 5. Pass. No violations.

Lebanon Valley Craft Brewery, 840 N. 7 St. Lebanon, Dec. 5. Pass. One spray bottle, a working container in the bar area, used for storing chemicals/cleaners taken from bulk supplies, was not marked with the common name of the chemical.

Trattoria Fratelli, 502 E. Lehman St. Lebanon, Dec. 5. Pass. Observed one scored cutting board not resurfaced or discarded as required. PIC discarded the cutting board. No sign or poster posted at the handwash sink in the men’s and women’s toilet rooms to remind food employees to wash their hands. Note: PIC stated that the signs were inadvertently removed when new towel dispensers were installed.

Dollar Tree #4232, 109 Northside Commons, Palmyra, Dec. 4. Pass. No violations.

Palmyra VFW Memorial Post 6417, 232 E. Main St. Palmyra, Dec. 4. Pass. No violations.

The Caffeination Station, 707 E. Broad St. Palmyra, Dec. 4. Pass. No violations.

Wellspan G.S. Courtyard Café, 424 Walnut St. Lebanon, Dec. 4. Pass. Observed deeply scored cutting boards not resurfaced or discarded as required. One container of utensils and one container of lids, both food contact surfaces, were observed to have food debris in the bottom and were not clean to sight and touch. The bottom of one refrigeration unit, a non-food contact surface is not cleaned at a frequency to preclude accumulation of dirt and soil. The facility has a pest control program; however, one glue trap observed appeared to be very old and had not been checked for several months according to the dates on it. Frequency of cleaning behind two refrigeration units is in adequate as evidenced by dirt and debris behind the units.
