HealthSmart in a Minute: Patient-Centered Medical Home
You may have heard the term Patient-Centered Medical Home – or PCMH.
Dr. Dennis Gingrich says the PCMH model puts the patient at the center – and partners them with their primary care doctor. They’re supported by a care team – which is made up of nurses – a pharmacist whatever the patient needs.
